Please download, fill out, and turn-in FIRST's "Release, Hold Harmless & Authorization to Reproduce Physical Likeness" form. This form is necessary for all FIRST participants, including team members, mentors and volunteers to participate in FIRST events. This form needs only to be turned in once.
Students are expected to wear team uniform consisting of a Team 122 T-shirt (must be purchased), blue jeans, sneakers, shiny blue/silver decorated hat (provided), and blinking LED astronaut necklace and/or blue and silver beads (provided), and boot cover for sneakers (provided). Some will wear shiny blue/silver tool belt (provided).
Students are not allowed to bring any computer games (GameBoy, X-Box, etc.). They may bring a CD player, iPod, and/or cell phone (to call family only - not friends).
CD players and iPods are not allowed during competition.
Have them get plenty of rest prior to leaving.
Students who become ill must report the illness to Ms. Talmage so a go/no go decision may be made in the best interest of the ill student and the team.
All school forms (Permission Form, Code of Conduct, etc.) must be filled out and signed. All students must have an emergency number to reach a parent/guardian. These forms need to be turned in prior to each trip.
Medical forms must be completed by a parent/guardian prior to the event. Permission to administer prescribed medication must be granted, as well administering over-the-counter medication (aspirin, cold & flu meds, etc.).
According to school policy, Students must ride the bus to and from the event. Written permission must be on file with the school allowing the student to ride with parent/guardian or someone else.
Parents/guardians may pack the students food for the trip and hotel.
Approximately $80 should be available to purchase Lunch and Dinner.
Parents are welcome to all events. If you wish to stay overnight, you must make your own arrangements and pay all costs. Sharon at Poquoson Travel (757-868-7052) is handling all travel plans for Team #122. Please contact her if you have any questions about the hotels, or to try and make reservations at the same hotel. (Room availability may be limited)
